About10-15%ofpatientswithcervicalcancersufferfromrecurrencedespitehistologicallynegativelymphnodes(pN0).Occultmicrometastasesorsmalltumourcellclustersmaycontributetodiseaseoutcome.TheaimofthisstudywastocompareattheRNAlevel2knowntumour-associatedgenes,HPV16-E6andcytokeratin19(CK19),asmolecularmarkersforthedetectionofdisseminatedtumourcells.Real-timereversetranscriptionPCRtechnologywasusedtoquantifygeneexpressioninhistologicallypositiveandnegativesentinellymphnodes(SLN)from70patientswithcervicalcancer.Lymphnodesfromnoncancerpatientswereusedascontrols.Calculatedcopynumberswerenormalisedtothegeometricaverageofthemoststablehousekeepinggenes.Weobservedagoodcorrelation(R=0.915)betweentheexpressionofbothmarkersinSLNwithhistologicallyconfirmedmetastases.However,markergeneexpressiondifferedconsiderablyinhistologicallynegativenodes:CK19transcriptsweredetectedin90of112SLN(80.4%),whereasonly38nodes(33.9%)werepositiveforHPV16E6mRNA.Inparticular,62of74SLN,whichwerenegativebyhistology,andHPV16E6mRNAexpressedCK19mRNA.Moreover,8of10lymphnodesfromnoncancerpatientsexpressedCK19mRNA.SystematicerrorsduetoRNAdegradationorincompletecDNAcouldberuledout.ItisconcludedthatHPV16E6mRNAismorespecificandmoresensitiveforthedetectionoftumourcellsinSLNthanCK19mRNA.ThespecificityofCK19islimitedbecauseoflowlevelexpressioninuninvolvedpelviclymphnodes.©2007Wiley-Liss,Inc.
